These are some commonly used forms that we have placed on our site for your convenience. Any questions regarding these forms, please contact Kate.

CPP & Old Age Security – These charts illustrate the various rates for both the Canadian Pension Plan and Old Age Security Benefits.

Full Power of Attorney - This form is used if you wish to grant someone other than yourself the ability to give investment instructions on your account. This form also grants the power of attorney the ability to withdraw funds from your account.

Limited Power of Attorney – This form is used if you wish to grant someone other than yourself the ability to give investment instructions on your account. This form limits the power of attorney to buy and sell instructions.

Power of Attorney Representation Form – This form is used to gather information necessary to add a Power of Attorney to your account at ScotiaMcLeod.

Home Buyers' Plan Request to Withdraw from an RSP – This form is used if you wish to withdraw funds from your RRSP under the HomeBuyers Plan.

KYC Summary Form – This form is used to open a new account at ScotiaMcLeod. It provides us with information necessary to open your account.

Transfer from an RSP or RIF to another RSP or RIF on Breakdown of Marriage or Common Law Partnership – This form allows us to transfer RRSP or RRIF assets to a spouse on a breakdown of marriage or common law relationship as per a separation agreement.

MKYC Individual – This form allows you to update various pieces of information on your account such as address changes, investment objectives, adding interested parties to your account etc.

MKYC Legal – This form allows you to update various pieces of information on your legal entity accounts (e.g. corporate account) such as address changes, investment objectives, adding interested parties to your account etc.

Transfer Authorization for Non-Registered Investments – This form allows you to transfer assets from another institution to your non-registered investment account at ScotiaMcLeod.

Transfer Authorization for Registered Investments - This form allows you to transfer assets from another institution to your registered investment account at ScotiaMcLeod.

Pinnacle Annual Review Form – This form is used for Pinnacle account holders. It verifies that an annual review has been preformed on your account and that you wish to maintain your current asset mix, or allows you to make changes to your current asset mix.

Request to Reduce Tax Deductions at Source – This form is used to reduce the income taxes withheld by your employer once you have made an RRSP contribution. This provides you with immediate income tax relief as opposed to waiting for the processing of your personal income tax return the following year.
